Augmented Reality Neurosurgical Navigation

Boston based Zeta Surgical develops surgical navigation + robotics tools to make minimally invasive surgeries easier, faster + more accessible. Currently, neurosurgeons heavily rely on neuronavigational systems in operating theatres during cranial procedures to ensure instrument tracking positioning against preoperative imaging, but this technology is barely used at the point-of-care, such as in outpatient & emergency settings. A major obstacle to improving point-of-care neurosurgical procedure accuracy with image guidance technologies is the lack of a rapidly deployable, real-time registration & tracking system for a moving patient. Zeta is a computer vision-based navigation system that provides real-time tracking of patients & surgical tools during procedures, starting with trauma cases such as with External Ventricular Drain (EVD) placement.

 
 

Augmented Neuronavigation

A novel one-cart mixed-reality guidance system designed for ICU/ER based neurosurgical procedures.

Existing neuronavigation systems utilised in operating theatres are often complex to operate and typically presented in a dual-cart format — this makes them slow to deploy and unsuitable for use in high traffic + space restricted environments such as ICU/ERs. Zeta is a novel one-cart solution with a small footprint designed specifically for rapid deployment in a wide range or surgical settings. The system is powered by Zeta's proprietary 3-layer augmented neuronavigational software. The mixed-reality software utilises on-board 3D cameras to track a patient + surgical tools and calibrates this with pre-operative data obtained from a patient CT/MRI scan which is displayed to the surgeon in real-time.

... Every year in the US, over 35,000 emergency EVDs are placed on patients suffering from the build-up of CSF leading to a life-threatening increase in pressure. Due to its emergent nature, EVD procedures are often performed freehand in Intensive Care Unit (ICU) and Emergency Room (ER) environments. Performing this procedure freehand may result in imprecise catheter placement which can cause damage to important neurologic structures.

The Zeta registration algorithm functions rapidly, dynamically, with sub-millimetre accuracy, eliminating a free-handed approach, fiducial markers & manual image-to-patient registration. Zeta guides surgeons through each step of the procedure, resulting in precise EVD placement in patients who are suffering from an emergent life-threatening build-up of fluid & pressure.


 

Commercialising Technologies

Zeta Surgical is a Boston-based MedTech startup founded by Harvard graduates Jose Amich and Raahil Sha. Zeta approached D+I to assist in the design and development of the navigation system, with a focus on creating a format and feature set that would enhance the usability and operability of the device in point-of-care settings such as ICU/ER environments. The system consists of; PC hardware and backup power systems; a touchscreen mounted to an articulating arm that allows the display to be positioned close to the neurosurgeon; a second articulating arm that extends from a central pole allowing the camera, tracker and drape attachment points to be positioned directly over the patient for optimum scanning parameters…

... In addition to its novel one-cart format other features include; the mechanical development of the systems articulating arms for optimum positioning (height/reach/balance); these touchpoints are protected by ergonomic aluminum handles; and integration of a drape attachment mechanism which improves operational workflow reducing time/risk in establishing a sterile field. The system is the first surgical device from Zeta Surgical — D+I developed a unique product design language for the Zeta brand, including a premium contrasting CMF that is representational of a high-value next-generation neurosurgical tool. Additionally, the solution required optimisation for ease of manufacture, assembly and serviceability. For Zeta, procedures such as EVD are just the beginning, the company believes its technology can help to democratize procedures by offering more precision and by lowering the barrier of entry.
